The Community Health Integrated Platform (CHIP) has introduced the "ASHA Digital Health Application (D2D Survey)" to collect and utilize data related to societal and health indicators gathered by frontline ASHA workers. This application facilitates resource allocation by healthcare administrators and coordination of primary healthcare services by health workers (ASHA, ANMs, and MOICs). It is officially approved by the Rajasthan government and endorsed as an innovation grant within the framework of the National Health Mission Project Implementation Plan by the Indian government.

Initially utilized by over 50,000 healthcare workers during the pandemic in Rajasthan, screening over 14 million beneficiaries, this application integrates health and community welfare aspects. It has been developed in collaboration with technical partner of the Rajasthan government, Khushi Baby (a self-help organization under Section 8), and is securely hosted on the Rajasthan State Data Center (RSDC) infrastructure by the Department of IT&C. The application is solely used by accredited governmental healthcare service providers.

Prior to data collection through this platform, explicit consent is obtained from all beneficiaries. The current version of the application primarily serves the following functions:

1. Conducting doorstep surveys for seasonal illnesses and ILI symptoms
2. Screening possible cases using a pulse oximeter and thermal scanner
3. Undertaking digital health surveillance based on responses (currently in Udaipur district)
4. Connecting families using Aadhaar-enabled identification
5. Linking families or individuals without Aadhaar to their demographic profiles
6. Manually entering embedded personal information in Aadhaar card QR codes without network connectivity
7. Performing larval surveys at the family level
8. Gathering information on distribution/treatment of medicines provided to families or individuals
9. Collecting population data on households and individuals
10. Gathering information on household characteristics
11. Collecting data on various NCDS and past illnesses
12. Gathering information on eligible women's pregnancy status
13. Maintaining GPS location in the background
14. Referring high-risk cases to health workers for immediate attention
15. Saving data offline and syncing upon network availability
16. Sharing data with relevant ANMs and healthcare workers CHIKITSA officials
17. Providing beneficiaries with educational videos for health awareness
